It is developed in 1962 by Bell Labs for reliability and safety analysis. 

Boeing's Dave Haasl acknowledged its value in safety evaluation in 1963.

Fault Tree Analysis (FTA) is for reliability and safety analysis, adopted first by Bell Laboratories.

What is Fault Tree Analysis?

It is a top-down detective technique using Boolean logic to analyze system failures through lower-level elements.

FTA is simpler than FMEA, as it focuses on all possible system failures caused by an undesirable event.

A Fault Tree Diagram (FTD) are logic block diagrams that display the state of a system in the state of its components.
